solated unilateral trochlear nerve palsy due to cavernous sinus meningioma is extremely rare. We report a case of right isolated unilateral trochlear nerve palsy caused by cavernous sinus meningioma that was successfully treated by gamma knife radiosurgery. A 68-year-old man presented with gradually progressing vertical diplopia. On neurologic examination, we discovered a superior oblique muscle palsy on his right eye and detected a cavernous sinus meningioma in brain imaging. After gamma knife radiosurgery, vertical diplopia was resolved.
